Finance has dropped off paper copies at the majority of departments throughout the Nation for members to pick up and complete. You can download the digital PCD FORM HERE: CLICK THIS LINK TO DOWNLOAD

Before submitting, please ensure:

Your PCD form is signed at the bottom.

Your banking information is provided if you require direct deposit (EFT).

If your banking has not changed, please indicate your bank name (CIBC, Scotiabank, etc.) so Finance can verify your existing information.

If you have a new bank account, please include your updated banking information.

Any required legal court documents are attached.
